As a Medical Home Care Advocate, you will support your assigned practices with on site care coordination to facilitate appropriate primary care visit planning, follow up and coordination in order to reduce adverse events for high risk/ Health Home cohort patients.
Responsibilities of the Medical Home Care Advocate include:
Working on site with assigned Medical Home practices and their clinical teams in managing cohorts of high risk/health home members to ensure they are seen by their PCP as needed and are meeting their goals
Working with the PCP to ensure that all aspects of the patient plan of care is being followed
Planning for each PCP visit, prepping the PCP and meeting with the patients before their scheduled practice visits
Following up with specialists to ensure referrals to specialist appointments are completed and specialist visit documentation is provided to PCP in a timely manner
Reviewing the Medical Home Registry updates daily to identify any unplanned ER visits or admissions for the high risk patients. This will include coordinating their return to primary care within 7 days.
Coordinating support for patients using available United care management ICCT resources as well as local resources for education and outreach
Collaborating with the practice Medical Home clinical team to identify and address barriers to closing current care opportunities
Collaborating with local hospital UM nurses and transition nurses for transitional needs of high risk patients within the Medical Home practices
Using the Medical Home Population Registry for high risk patients in order to plan visits, track follow up and ensure open care opportunities are addressed
Reporting daily, weekly and monthly the measurable progress on high risk patients to the clinical teams as well as at JOC

Active and unrestricted RN License in LA
Minimum 3 years working within a practice setting as part of the clinical team
Strong written and verbal communication skills
Ability to work as a team
Computer proficiency with MS Office: Word, Excel and Outlook
Experience caring for patients with chronic conditions
Office management experience
Guided Care Nurse Certification*

- To become recognized professionally as a Guided Care Nurse, a candidate with a nursing degree and a current license must complete an accredited online Guided Care Nursing course. Upon completion of the course, the nurse is eligible to take an online examination leading to the American Nurses Credentialing Center's new Certificate in Guided Care Nursing.
Certificate is not a certification, but rather a one-time recognition of professional achievement.
