Company Description For many older adults in need of long term care services, home is their first choice for care. VNSNY CHOICE, a comprehensive managed long-term care program, enables over 3,600 serving frail older adults in New York City to satisfy that preference.VNSNY CHOICE was established in 1998 as an innovative program of integrated care. Under the program model, care managers, or 'Nurse Consultants', lead an interdisciplinary team that also includes social workers, rehabilitation therapists, nurse practitioners and nutritionists. These clinical specialists collaborate closely with program member/patients, caregivers, community physicians, and other providers to achieve a common goal: helping member/patients remain independent at home and in their communities for as long as is safely possible.We are seeking talent for the opportunity described below.

Responsibilities Responsible for providing initial and on-going support/instruction to the VNSNY CCC Interdisciplinary Team including Regional Program Directors, Rehab Consultants, Team Managers, and other Clinicians. Ensures full integration of Rehab staff into their role through initial hiring, ongoing supervision and continuing education. Acts as an expert resource to Rehab/clinical teams related to policy, workflows, guidelines, clinical issues, problem resolution, complaint resolution and other clinical or service related issues. Covers multiple regions.

Works under general direction. Qualifications Licensure: Current registration to practice as a Licensed Physical Therapist in New York State, required.Education: Bachelors Degree in Physical Therapy required. Masters or Doctorate Degree in Physical Therapy or health care field preferred.Experience: Minimum of three years experience in home-care physical therapy, required. Experience in home-based long-term care required.

One-year experience as an Adult Educator, OR two years experience as a Rehab Consultant required. Effective oral/written communication and interpersonal skills, required. Minimum of five years progressive experience in physical therapy, preferred.
