Essential Functions: Completes assessments and makes appropriate level of care referrals to Norfolk Community Services Board programs and services. Other responsibilities include: coordinating client care, treatment planning, case management, and maintaining accurate records.
Education & Experience: Requires a VA licensed professional plus considerable related experience. Must possess knowledge and skills in working with adults with co-occurring mental illness and substance abuse issues. CSAC preferred.
