DESCRIPTION

Provides adequate in-community support for Snoqualmie Health and Human Services program specifically medical transportation for Snoqualmie Tribal Members and Medicaid eligible program clients.

DUTIES

To perform with job successfully, an individual must be able to perform each essential duty satisfactorily. Reasonable accomodations may be made to enable qualified individuals with disablities to perform the essential functions.

  • Provide transportation support for SBH clients, Snoqualmie Tribal members and others who are Medicaid eligible, under the direction of the SBH Director and Transportation Liaison.
  • Maintain standards of confidentiality.
  • Maintain all current standards of documentation, for travel, mileage, expenditures, and related reporting requirements, within specific deadlines.
  • Excellent customer service skills required.
  • Must be present, respectful, and participate in the various Snoqualmie Indian Tribe culture events, trainings and ceremonies and work closely with parents, staff and communities to help build healthy and positive relationships.
  • Work in a semi-structured work week, allowing flexibility for evening and weekends when necessary.
  • Attend SBH staff meetings and trainings as appropriate and attend continuing education as required and/or needed to fulfill job duties.
  • Assist in program planning, transportation reporting, maintaining logs and related documentation for Medicaid covered services.
  • Program planning, policy development, Medicaid outreach and referral and/or interagency coordination related to Medicaid and Non Medicaid covered services.
  • Promote and provide information and referral to clients related to smoking cessation and problem gambling to assist them in leading a healthier lifestyle free from the effects of smoking and addiction.
  • Upon hire, position requires immediate completion of Fire Suppression training, CPR/First Aid training, Passenger Assistance and Sensitivity Training and Child Safety Seat Securement Training.
  • All other duties as assigned.

SKILLS & ABILITIES
Education: High School Graduate or General Education Degree (GED) desired.
BA in related field desired

Experience: Experience working in Native Community with families and parents desired.
Previous driving experience required.
Minimum age requirement is 23 years.

Computer Skills
Proficient in computer skills such as MS Word, MS Excel, and the Internet.

Certificates & Licenses
Current CPR, First Aid and TB Immunization with valid documents.
Maintain a valid Washington State driver's license.
Must be able to successfully pass the Tribe's Defensive Drivers Class.
Must be able to complete and pass FBI Background check.
Must complete and pass Washington State Motor Vehicle Background Report.

Other Requirements
Must be 23 years of age or older
Must be able to pass background check.
Must be drug free and pass random drug testing.
Must maintain confidentiality per HIPPA.
Must demonstrate ability to communicate respectfully and work well with children, families, staff and community.
Demonstrate ability to work with diverse tribal groups or communities.
Knowledge of American Indian culture, history, and healing ceremonies.
