Santé of N. Scottsdale

is seeking a dynamic, experienced, team-oriented and quality focused professional to assume a role Guest Services Assistant.

Guest Services is responsible for assisting with all guest services programs within the facility. The Associate assumes responsibility, and accountability of supporting the guest service department while on duty.

General Responsibilities:
Conducts facility site/show tours as needed.

Lead contact for taking inquiry phone calls and referral calls. Inputs all inquiry information into IRM Point Click Care (PCC).

Interviews guests/families as necessary, and provides clinical, financial, and special needs/requirements information to Santé team members regarding pending admissions. Coordinates with Rehab Liaisons, Community Relations, Business Office Management and Care Management on review, acceptance or denial of all admissions and coordinates onsite assessment visits if necessary.

Plans, updates and distributes the daily meeting census report and the room roster information as needed.

Assigns Care Managers and arrival guestrooms for each incoming admission.

Processes and admits guests from IRM into active in PCC.

Assists with completion of all admission paperwork and input of all fields for PCC and then scans all pertinent documents and cards.

Maintains teaching information files for admission packets, and provides and completes packets for new admissions as needed.

Arranges transportation with Santé Valet or external transportation services for admissions.

Participates in discharge planning, development and implementation of social care plans and patient assessments as needed.

Provide information to patient/families as to Medicare/Medicaid, and other financial assistance programs available to the patient.

Develop and maintain a community and social services referral file of agencies and organizations that provide assistance to patients.

Working Conditions and Physical/Sensory Requirements:
Works in office areas as well as throughout the facility.

Moves intermittently during working hours.

Is subject to frequent interruptions.

Works beyond normal working hours, on weekends, and in other positions temporarily, when necessary.

Is subject to call back during emergency conditions (e.g., severe weather, evacuation, post-disaster, etc.).

Is subject to exposure to infectious waste, diseases, conditions, etc., including TB and the AIDS and Hepatitis B viruses.

Maintains a liaison with other department supervisors to adequately plan for Guest Services/activities.

Must be able to read, write, speak, and understand the English language.

Must possess sight/hearing senses or use prosthetics that will enable these senses to function adequately so that the requirements of this position can be fully met.

Must function independently and have flexibility, personal integrity, and the ability to work effectively with patients, personnel, and support agencies.

Must be able to relate to and work with ill, disabled, elderly, emotionally upset, and, at times, hostile people within the facility.

Must be able to push, pull, move, and/or lift a minimum of 25 pounds to a minimum height of 5 feet and be able to push, pull, move, and/or carry such weight a minimum distance of 50 feet.

Skills, Knowledge, or Ability:
Must demonstrate the knowledge and skills necessary to provide care appropriate to the age-related needs of the patients served.

Must be a supportive team member, contribute to and be an example of team work and team concept.

Must be knowledgeable of the rules, regulations, and guidelines that govern nursing care facilities.

Must possess the ability to make independent decisions when circumstances warrant such action.

Must have patience, tact, cheerful disposition and enthusiasm, as well as be willing to work with patients based on whatever maturity level in which they are currently functioning.

Minimum Qualifications:
Minimum of one (1) year experience with either: admissions in a Skilled Nursing Facility, or hospital discharge, or case management required.
