Responsible to Managing Director of Hospice Clinical Operations Description Responsible for the overall clinical management process and performance improvement activities for the region assigned. JOB DUTIES/KNOWLEDGE 1. Assists in the development, implementation and evaluation of the organization’s programs for the improvement of quality and service productivity. 2. Educates each agency Administrator and Director of Nursing on the Performance Improvement collection tools and submission requirements. 3. Performs Clinical Management reviews during agency visits and develops site specific Performance Improvement action plans to address the agencies’ identified needs. 4. Identifies teaching needs based on the Clinical Management site surveys, monthly-focused review results or agencies’ stated needs. 5. Conducts in-services on the teaching needs that have been identified during the Clinical Management process (see process stated above). 6. Implements and assists in the evaluation of the regions’ compliance to state and federal regulations. Ensures regional preparation for certification and accreditation surveys. Involves regional staff at all levels in survey compliance issues. 7. Serves as regional resource to all staff regarding Performance Improvement and documentation. 8. Coordinates any Amedisys resources that have been identified as a Clinical Management need to the specific agency (Example: Operations assessment of personnel files). 9. Coordinates and supervises orientation related to documentation and quality improvement for all new regional administrative employees. 10. Oversees the agency Performance Improvement program and ensures that all Performance Improvement data is completed and submitted on a timely basis to the corporate Clinical Services Department. 11. Performs meetings on a quarterly basis with the Director of Operations to update them on the status of their Performance Improvement program. 12. Reports any data submission complications to the Vice-President of Clinical Operations. 13. Attends Performance Improvement workshops and other educational activities to keep abreast of the latest information on quality issues. 14. Other duties as assigned.

Requirements
QUALIFICATIONS 1. Bachelor’s degree in nursing preferred. 2. Registered Nurse licensure. 3. Nurse Licensure Compact (NLC) documents the following: The mutual recognition model of nurse licensure allows a nurse to have one license (in his or her state of residency) and to practice in other states (both physical and electronic), subject to each state’s practice law and regulation. Nurses must legally reside in a NLC state to be eligible to have a multi-state license. See list of Nurse Licensure Compact States 4. Minimum of two (2) years of home health experience. 5. Expressed interest in details of documentation and quality. 6. Prefer Performance Improvement experience with knowledge of ACHC and HCFA standards. 7. Demonstrated knowledge of the appropriate skills for communicating at all levels within the organization.
