Initiates the nursing process on elective admissions to the hospital. Under the direct supervision of the Director of Admission Services within established policies and procedures, the incumbent performs the following functions: Gathers and transcribes physician’s orders when received in advance of admission. Obtains orders for the following day. Prepares patient chart and identifies nursing diagnosis. Ensures maintenance of patient chart and proper routing of chart. Interviews patient and/or family to obtain a comprehensive history. Identifies patient care problems and initiates plan of care based on assessment. Identifies potential discharge planning problems and based on patient’s developmental status, behavior, cultural and health concerns ensures multidisciplinary referrals are made. Provides education based on patient and family’s assessed needs. Completes pre-op teaching form and notes patient and family'’ level of understanding. Distributes patient information booklet and explains hospital rules and regulations, patient rights, and advance care directives, and reviews valuable procedures. Obtains vital signs, applies identification band, and carries out any ordered testing. Reports to assigned nurse on nursing unit. Practices safe transport of patients. Obtains all consents based on hospital Administrative policy. Provides for accurate and timely bed assignment by maintaining and updating hospital census. Performs charge duties as assigned. Receives, logs and secures valuables. Counts and releases upon discharge and reports discrepancies. Verifies accuracy of registration data and receipt of pre-certification and pre-authorization. Reviews admission consent forms for completeness. Obtains any missing documentation or information. Completes and refers to Departmental Conferences. Maintains adequate continuing education credits, licensure, as well as BLS certification. Participates in Admissions Quality Management Program. Practices infection control through adherence to department and hospital policies and procedures. Participates in appropriate role in fire and disaster drills, fires, and other emergency situations.
Education: Graduate of an accredited school of nursing. Bachelor of Science or equivalent experience preferred.
Experience: Two years experience as a staff nurse. Computer skills preferred.
Licensure: Current RN License in the State of New Hampshire. CPR certified.
