Responsible for the provision of direct client care: Provides welcoming atmosphere, ensures patient identity and confidentiality when providing services, administers appropriate decanoate injections as ordered, provides on-site urine drug screens as ordered, provides concurrent documentation of services, notifies dispensing pharmacies of lab results for clozapine dispensing, and notifies prescriber of concerns.

Asks the client to confirm identity with picture ID (if client is not well-known).

Assures client confidentiality while obtaining current information on their current mental status:

? current thoughts of suicide/homicide

? audio/visual hallucinations

? sleep patterns

? concerns that the client may have

? reports unusual findings immediately to ARNP, physician, or Medical Director.

Provides injections of decanoates as ordered:

? Compares current order to the medication available in the injection office.

? Determines site of last injection to provide site rotation.

? Prompts client to fill prescriptions or attend prescriber appointments.

? Documents interaction on Nursing Progress Note and fills out billing information.

? Documents injection on the Communication/Injection log

Provides on-site urine drug screening as ordered:

? Accompanies client to toilet facility and instructs client in use of cup.

? Follows directions for interpreting results.

? Reports results to prescriber.

? Documents interaction on Nursing Progress Note and fills out billing information.

Assists in the evaluation of clients with hypertension reported by the Nursing Assistant:

? Re-evaluates blood pressure comparing arms, if possible.

? Determines co-existing signs: headache, diplopia, tinnitus, nausea, weakness/numbness.

? Reports findings to appropriate prescriber.

? Calls 911 as needed (notify Risk Management with Unusual Occurrence Report)

Performs finger stick blood sugar analysis, when appropriate, and documents interaction on a Nursing Progress Note.

Assists as needed communicating with clients who have called the Med Hot Line.

Assists in completion of documentation:

Monitors status of reported labs daily:

? Print results and provide medical record numbers.

? All results are forwarded to the prescriber who ordered them:

o If all results are WNL, results may be provided without the client's chart.

o If any result is outside range, provide results with the client's chart

? CBC results ordered on a routine basis for clients receiving clozapine:

o Calculate ANC and graph results

o Send results to responsible pharmacies

o Post results on the clozapine registry webpage

o Provide results to the medical staff for review. Notifies medical staff immediately if results indicate that clozapine should be discontinued.

Documents "no-show" or "reschedule" on the Communication/Injection log as needed.

Initiates prior authorization forms in response to requests from dispensing pharmacies and routes them to the appropriate prescriber or the medical director for completion. Faxes completed forms to the appropriate third party.

Initiates discharge paperwork for clients who do not attend initial evaluation appointments or who have had three or more "no-show" for scheduled appointments or who have not been seen by medical services in six months or more and do not have a scheduled appointment.

Signs treatment plans as LMHP.

Documents outdated or unused medications that are placed in Chemical Waste container for pick up by outside vendor.

Provides Employee Health Services: Reviews current vaccination status for Hepatitis B vaccine and tuberculin skin testing and administers them as appropriate, determines if there is a latex allergy or other concern which may require further inquiry.

Determines eligibility for tuberculin skin testing and administers TST as appropriate.

? Determine if previously positive.

? Determine current cluster of symptoms.

? Inform of current recommendations for assessing and treating positive skin tests.

Reviews basic information about Standard Precautions and Transmission-based Precautions.

Reviews information about Hepatitis, discusses Hepatitis B vaccination eligibility, and reviews consent form for Hepatitis B vaccine. If employee requests the series, ensures the first injection is given within the first ten days of employment. Follows employee to ensure vaccination series completed in a timely fashion (provided the employee has not separated employment).

Supervision of nursing assistants assigned to Medical Services: monitors attendance and activities providing feedback as needed to ensure high quality service. Assists as needed with vital signs, assessments of reported concerns, and patient well-being calling 911 as needed.

Customer Service:

Greets clients as soon as possible after check-in and provides welcoming atmosphere.

Provides confidentiality by closing the door and modulating voice during assessment/injection.

Keeps personal conversation quiet to avoid being misinterpreted by nearby clients.

Treats all customers – clients, staff, other departments – with courtesy.


Documents clearly and effectively utilizing appropriate forms.

Seeks information as needed.

Attends staff meetings.

Coordinates breaks with other staff members as appropriate.

Requests time off with appropriate notice.

Notifies the supervisor and executive assistant as soon as possible if calling off for illness or emergency.

Team Work:

Orients and mentors new staff members – nurses, ARNPs, physicians, nursing assistants – as needed and as appropriate.

Considerate of other team members. Channels questions, suggestions, and constructive criticism appropriately. Works with the team to identify opportunities for improvement.

Minimum Qualifications:


Graduation from a program leading to Licensure and current clear and active license

Language Skills
