As the HIV/AIDS Coordinator at Community Health Center Inc., you will:
Be Responsible for the development of a strategic outreach plan which will provide information to provider agencies and clients about our services.
Assist in the design and update of multi-lingual marketing materials for dissemination
Recruit volunteers and client to disseminate materials and help with community awareness events.
Conduct HIV/AIDS C&T, education, CRCS and HPC throughout Middlesex County in accordance with Department of Public Health policy.
Maintain a caseload of 12 CRCS clients and 6 HPC clients on an ongoing basis.
Provide intensive case management to clients who exhibit high risk behaviors.
Enter all client data on a daily basis and provide data reports to supervisor on a monthly basis.
Prepare quarterly narrative and data reports for submission to DPH detailing the progress on grant goals.
Distribute condoms and provide rapid HIV/HCV testing as part of outreach throughout Middlesex County.
Participate in any community event that could provide a forum for promoting HIVAIDS education, testing and care at CHC.
Work with contacts at community agencies in the CHC. service area to arrange information sessions.
Maintain membership on the Ryan White Planning Council and other community consortia.
Participate in department activities and events whenever possible.
Attending weekly staff meetings.
Perform other duties as assigned.
Bachelors degree in a related discipline (Human Services, Psychology,etc) required
CT HIV/AIDS Education Certificate or willingness to acquire
CT HIV/AIDS Counseling and Testing certificate or willingness to acquire
Knowledge of HIV/AIDS, Hepatitis C and other sexually transmitted infections and the ability to communicate facts efficiently to diverse populations.
Strong communication, organizational, time-management, prioritization, multi-tasking, and interpersonal skills
Experience with intensive case management
Proven experience in community outreach
Professional responsiveness and adaptability to rapid change
Able to take initiative when needed and also work as a team on a regular basis
Demonstrated proficiency in Microsoft Office and internet related applications
Excellent oral and written communication skills
Valid state of Connecticut driver's license with good driving record
