Under the direction of the Regional Manager, the Care Management Supervisor provides the daily leadership, management and mentoring of the Care Management staff that serves our members. They develop and monitor policy and protocol, resolve member issues, and assist the organization in developing, implementing, and maintaining the Family Care program. All of this is done to support the organization’s dedication to providing accessible, cost-effective, high-quality, person-centered care that puts the member first. They will perform these duties personally or through the work of their staff.

Goal & Objective: Ensures that quality care is delivered in a consistent manner in adherence with the Family Care contract and Northern Bridges policies and procedures.

Demonstrates an understanding of related policies and procedures.
· Ensures policies and procedures are implemented within their hub in accordance with the organizational implementation plan and monitors regularly to ensure ongoing adherence.
· Identifies, evaluates, and implements measurement tools for critical timelines.
· Contributes to and leads the development of organizational projects and work groups.

Ability to develop and communicate performance expectations through regular 1:1 meetings with team members.
· Holds team members accountable to meet required timelines in accordance with contractual guidelines.

Necessary Training and Experience

NorthernBridges expects employees to understand and to incorporate the values of our organization in their day-to-day practice. While ensuring that statutory parameters are satisfied, the organization is willing to consider a combination of the following education, training and experience requirements, which provides the necessary abilities to perform the duties of this position.
· NorthernBridges Care Management Supervisor must have a bachelor’s degree in human services or related field OR a minimum of two years of supervisory/leadership experience.
Ÿ There is a strong preference for candidates with experience in case management, or those with certification or licensing in social work or nursing.

Family Care concepts, protocols, and regulations o
Northern Wisconsin health care systems and long-term care networks o
Utilization management, quality improvement, managed care concepts, marketing, and program development

Ÿ The Care Management Supervisor may be required to travel to other locations, and therefore will be required to hold a valid driver’s license and have access to reliable transportation.

Work Environment

The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job. The position normally works in an office environment where the noise level is usually quiet. Hours of work will generally be during regular business hours and average at least 40 hours a week. There will be some variation in work hours due to special projects, deadlines, and other concerns. Occasional out-of-town and overnight business travel is required for business and education, by air and auto. This position is in the No Risk Exposure category for blood borne pathogens and will follow all recommended training and procedures for this category.
