Qualifications

JOB SUMMARY

The Patient Navigator functions on the multidisciplinary team as an advocate, interpreter, educator, and counselor for oncology patients.

They are responsible for facilitating quality and comprehensive services adult patients with an oncology diagnosis.

They will coordinate patient care throughout the continuum in collaboration with the multidisciplinary

team and serve as a clinical resource with expertise in hematology/oncology care management.

They will serve as a liaison throughout the facility and in the community regarding services provided for the oncology patient population.

They will have the ability to provide quality nursing care, inclusive of: direct clinical practice, consultation,

education and research resource referral

Follows all established organization and department policies, procedures, objectives, and quality assessment. Responsible for assisting in maintaining a safe working environment throughout the organization by adhering to all safety management policies and procedures which reflect the mission and philosophy of Ascension Health System. Attends all mandatory in-service programs and maintains professional and education requirements.

EDUCATION REQUIRED

Graduate of an accredited school of nursing

Completion of a formal Navigator program will be requried within 1 year of entering this position

Oncology Nursing Society (ONS) course completion required within 6 months of entering this position

EDUCATION PREFERRED

Bachelors in Nursing

REGISTRATION, CERTIFICATION AND/OR LICENSURE REQUIRED

Current RN license in state of IN

BLS

REGISTRATION, CERTIFICATION AND/OR LICENSURE PREFERRED

ACLS within one year of entering this position as specialty area dictates

EXPERIENCE REQUIRED

One year recent experience working in specialty area (Oncology, Breast, Cardiovascular, etc as dictated by specific job opening)

EXPERIENCE PREFERRED

Three years experience working in specialty area (Oncology, Breast, Cardiovascular, etc as dictated by specific job opening)

SKILLS, KNOWLEDGE AND ABILITIES REQUIRED

Excellent human relations and oral/written communication skills

Self- directed with the ability to organize and manage task and projects.

Ability to pay careful attention to detail required

Ability to promote programs and services to the community.

Must demonstrate appropriate knowledge of the principles of growth and development

over the life span of the patient as well as extensive knowledge of the oncology disease process and related interventions.

Must possess the ability to assess patient data relative to age specific needs and provide

care as described in the department┬┐s policies and procedures
