The individual in this position assists the Outreach Coordinator in educating the community about chronic disease management and healthy lifestyles, and connecting the community residents with resources.
Frequent contact with physicians’ office staff, and community (individuals, businesses and groups). Involves distribution of materials and verbal communication about chronic disease management and prevention.
MINIMUM EDUCATION REQUIREMENTS
High school education or equivalent.
MINIMUM YEARS OF EXPERIENCE REQUIRED
Prior experience in community outreach or related work is beneficial.
REQUIRED LICENSE, REGISTRY OR CERTIFICATION
Valid NJ driver's license required. Certification, or certification in medical translation required.
SPECIAL KNOWLEDGE, SKILLS AND/OR ABILITIES REQUIRED
Familiarity with the Hunterdon County area and community organizations helpful. Bi-lingual in English and Spanish.
DESIRED OR PREFERRED KNOWLEDGE, SKILLS AND/OR ABILITY
Must be community oriented and comfortable speaking about chronic disease-related education, screening, testing and follow-up.
