DESCRIPTION: The Population Health Data / Quality Coordinator promotes individual and population health by leading FQHC informatics and quality improvement initiatives across all units within SPLG’s service area. Responsibilities include developing, implementing, and evaluating these initiatives, establishing, leading and maintaining intra- and inter- departmental teams, and managing work plans and timelines. Work will involve multiple initiatives and projects at any given time and will change as the initiatives and projects evolve. Specific focus in supporting the QA/QI Plan, PCMH certification, Clinic Meaningful Use guidelines and UDS reporting are priorities.The Population Health Data / Quality Coordinator provides all aspects of administrative leadership and support in the development, implementation and maintenance of Patient Centered Medical Home (PCMH) status within Salud practice sites. This position provides the informatics oversight, education, training and administration required to successfully implement PCMHs in various Salud practice sites. This position reports to the Chief Operating Officer (COO).

MINIMUM QUALIFICATIONS:
Bachelor Degree in business administration, health care administration/IT or nursing is required. Master's degree in business administration or healthcare administration is preferred.
Minimum of two (2) years experience in a medical practice site leadership position (multiple sites preferred).
Strong project management skills.
Proven experience developing and writing medical policies and procedures.
Knowledge of electronic medical record and billing systems.
