The Foster Boarding Home Intake Department of Edwin Gould Services for Children and Families (EGSCF) is seeking a dynamic and experienced Case Planner to ensure that all children and families are effectively and safely provided with services to quickly address the reasons for placement and begin working toward family reunification. Case Planner will work out of our Brooklyn office located at 151 Lawrence Street. This candidate should have a genuine love for working with children, teens, families and foster families. This individual will be responsible for:
Case management services for children in Foster Care; develops, communicates and implements individualized service plan and treatment program which responds to identified needs of each child and natural/foster family; engages the child and family in setting goals, establishing priorities and implementing service/treatment plans
Ensure compliance with ACS and agency policies and procedures, including time submission of documentation (i.e. Progress Notes, FASP’, PHR’s and Court Reports); conduct field work including home visitation, court appearances, school/hospital visits, as well as escorting and advocating for clients; attend and participate in IOC(Improved Outcomes for Children) Family Team Conferences
Undertakes activities i.e. advocacy and referral to facilitate access to services needed, including but not limited to health services, education, day care, housing, drug and alcohol rehabilitation, mental health services, vocational guidance, employment and financial entitlement

Skills
The candidate should have a working knowledge of guidelines, policies and regulations related to child welfare, safety, permanency, and well-being. Proficient knowledge in the use of NYS Connections Database. Bilingual REQUIRED (Spanish/English).
Notes
Two years of child welfare experience and two years of direct Foster Care experience (preferably Intake Department) REQUIRED; Ability to manage multiple deadlines; Candidate must also possess excellent written and verbal communication skills; as well as excellent organizational and interpersonal skills. Ability to work with diverse populations, process and respond appropriately. Candidate must be willing to travel and willing to work flexible hours and or days (Required)
