Responsibilities include budget,supervision ,LOS,creating strong relationships in the community, and working in partnership with Case Management and other health care professionals to assure coordination of care for patients and families. Oversight of 33 direct and indirect reports Medical Social Worker who provide consultation to patients / families regarding social, environmental, psychological, financial and cultural factors related to illness and hospitalization supports continuity in care management, and organizes all aspects of discharge planning.
Masters prepared LCSW with at lest 3-5 years of supervisor and acute care experiance.
