Reporting to Intake Supervisor, responsible for the intake and disposition of all incoming client/patient referrals and for review and verification of insurance reimbursement in compliance with agency policies and procedures.
graduate of accredited RN program
1 - 3 years nursing experience
RN license - NH or other Compact State
BCLS within 30 days of hire
Accurately assesses and completes referrals for agency services in an accurate, timely manner according to agency processes, and refers questions to appropriate Manager/Director as needed.
Coordinates the flow of information among the appropriate parties including agency staff, the referral source, the client/patient and physician.
Acts as a liaison and a resource to physicians, clients/patients, families, discharge planners and others regarding services offered by the agency.
Provides management, coordination and communication of patient care orders, physician license verification, Medicare system and E-Discharge according to agency processes.
Verifies insurance coverage, obtains authorization, and communicates this information to clinical and billing staff as needed.
Maintains established agency policies and procedures, objectives, quality improvement programs, safety/environment of care programs and infection control standards.
Assists with triage of patient/family/physician/community phone calls.
Enhances professional growth and development through participation in educational programs, current literature, inservices, meetings and workshops.
Supports patient's rights, responsibilities and professional code of ethics.
Demonstrates ability to perform all assigned functions related to the Intake Department or other duties as assigned according to established procedures.
