Care Manager- Emergency Room 1100-2130
4 ten hour shifts each week
Schedule: Su/Wed/Thurs/Fri then Mon/Tues/Wed/Sat

Management for the assigned inpatient Care Management population. This position is designed to facilitate an effective process of the new care model; supporting quality patient care, safety and financial components; promoting integration of a seamless care model; coordinating patient throughput; collaborating to include coordination as evidenced by metrics, optimizing performance and adoption of best practice. Performs duties and responsibilities in a manner consistent with our mission, values, and Mercy Service Standards.

Care Manager II (Pay Grade 16):

Minimum Qualifications
Education: Graduate of an accredited School of Nursing, Minimum Required Bachelor of Science in Nursing, Required
Experience: 2-3 years acute care hospital setting, Minimum Required
Licensure: Current License in the state of employment, Minimum Required
Certifications: BLS (CPR) at hire date, Minimum Required
Other: Strong verbal and written communication and facilitation skills, Minimum Required Strong assessment skills, time management skills, critical thinking skills, Minimum RequiredAdvanced computer skills, Minimum Required Must have ability to interact effectively with a variety of people and situations at all levels of the organization, Minimum Required

Experience: Care Management or Utilization Management experience, Preferred
Certifications: Certification in Case Management, Preferred

Care Manager I (Pay Grade: 15):
MINIMUM REQUIRED
Education: Graduate of an accredited School of Nursing. Associates Degree of Science in Nursing.
Licensure: Current License in the state of employment.
Experience: at least 2 years of acute care RN experience in a hospital setting.
Certification/Registration: BLS (CPR) at hire date.
Other skills & knowledge: Strong verbal and written communication and facilitation skills. Strong assessment skills, time management skills, critical thinking skills. Advanced computer skills. Must have ability to interact effectively with a variety of people and situations at all levels of the organization.
