Under the general direction of the Director, Critical Care Services and Clinical Leaders and in accordance with:

  • standards of care and practice established by the Nursing Organization and, the specific department;
  • the rules, regulations and/or standards established by the State Board of Nursing, the Joint Commission for Accreditation of Healthcare Organizations, and appropriate specialty organizations relevant to the patient population under care, the Clinical Nurse I performs the following functions:
ESSENTIAL FUNCTIONS Supporting the SICU/CCU primary care model of patient care delivery the RN performs the following:

Patient Care

Assessment: Interviews the patient and/or family to obtain a comprehensive history and to obtain data about patient’s development status, behavior, cultural, and health concerns. Examines the patient to obtain objective physical data. Reviews clinical data from diagnostic studies and previous records. Completes a physical and psychosocial assessment based on interview, examination and review of clinical data. Based on assessment, identifies nursing diagnoses/patient care problems. Documents data and assessment on appropriate form according to instructions. Collaborates with the multidisciplinary team to address patient needs. Reassesses patient status at established time frames and as indicated by the patient’s status. Reports status and changes in status as appropriate. Assesses potential for development of crisis situations and asks for assistance.

Planning: Develops an individualized approach to deliver nursing care to the patient and family, including age-specific developmental needs and incorporating their input and preferences whenever possible. Establishes goals with the patient and the family. Collaborates with the multidisciplinary team to develop an integrated plan of care. Initiates discharge planning as an integral part of the plan of care at the time of admission and updates status of the discharge plan on a continuing basis. Documents plan of care appropriately.

Education: Graduate of an accredited school of nursing.

Experience: A graduate nurse. Licensure/Current registration in the State of New Hampshire as a Registered Nurse
