Community Health Centers of Burlington is recruiting for a 32-40 hour per week, 1 year grant-funded Housing Case Worker that is responsible for working with individuals and families who need support to establish and maintain stable housing, through case management and collaboration. This position will pursue housing options in the community with the client and then continue to work with them once housed to ensure ongoing stability and skills necessary to maintain independent housing.
Bachelor Degree in Social Work or other human service related field. MSW preferred.
A minimum of 1 year of direct human service experience.
Ability to meet clients "where they are" literally and figuratively, working with a diverse population of clients with a variety of challenges.
Strong communication skills
Strong knowledge of community services or the willingness and ability to do the work necessary to find information and services for the benefit of the client.
