Baystate Health is an integrated health care delivery system serving a population of nearly 1 million people in western Massachusetts. Baystate Medical Center is the Western Campus of Tufts University School of Medicine, has the only Level 1 Trauma Center in the region and the only neonatal and pediatric intensive care units.
Under the direction of the Director of YEAH! (Youth Empowerment Adolescent Health) Network, will outreach, engage and monitor pregnant and parenting students into the program by conferring with parents/guardians, teachers, school counselors, and school administrators; educate participants to overcome personal, social, and/or behavioral problems affecting educational performance; encourage participants to explore learning opportunities and persevere through challenges, connect participants with mental health professionals when needed, and provide information about degree programs, college admission requirements and financial aid opportunities to college, trade, technical and apprenticeship programs.
High School diploma required. Bachelor’s degree preferred. Passionate about making an impact for the greater good of the community and working with teen and young adult populations. Team player, positive attitude, self directed, able to manage multiple tasks, and prioritize. Able to follow through and give feedback. Knowledge of high school graduation or GED requirements, career counseling and guidance in the Springfield School System. Excellent communication and intra / interpersonal processing skill. Problem solving skills. Organizational Skills. Must be able to work independently and with a team. Strong computer skills.
Per diem position Monday - Friday 1:00 pm – 6:00 pm
Location 280 Chestnut St. Springfield, MA
Grant Funded position - 3 years
