Community Counseling Center, a leading nonprofit provider of family services and behavioral health, is seeking a full time Case Manager for the Adult Community Integration Program The program provides individualized services for adults with mental illness to maintain and improve their quality of life.

The Case Manager is responsible for providing timely and appropriate diagnosis, assessment, client-driven treatment planning, and effective coordination and referral of clients to appropriate community services. The position requires some local travel to see clients.

In addition to providing high quality family, individual and group therapies to help children, adults and senior citizens from all incomes and backgrounds, CCC offers a diverse array of programs.

CCC is a member of the National Council for Community Behavioral Health, the National Child Traumatic Stress Network, the Maine Association of Nonprofits, and is a United Way Partner Agency.

Minimum of one year of experience in one of the following areas: psychology, behavioral health, social work, special education, counseling, rehabilitation, nursing or a closely related field. MHRT/C Eligibility is required.

Bachelors Degree from an accredited institution with a specialization in psychology, behavioral health, social work, special education, counseling, rehabilitation, nursing or a closely related field. Experience working with adults and older adults, familiarity with community resources and elder services desirable.

Competence in the areas of assessment and case coordination.

Knowledge and willingness to abide by the National Association of Social Workers code of ethics and, where applicable, other relevant codes of ethics.

Active MHRT-C certification required. Active Maine LSW licensure preferred but not required.
