We are currently looking to add a dynamic Patient Services Coordinator to our team.

The Patient Services Coordinator is responsible for the scheduling of all unassigned patient/client visits in accordance with the Plan of Care and Physicians Orders. Ensures that all clinical field staff is scheduled to productivity every week. Responsible for processing all missed, rescheduled, reassigned, and declined visits. Supports and acts as a back up to the Clinical Field Support Specialist as needed. The Patient Services Coordinator reports directly to the Director of Operations.

Essential Duties and Responsibilities:
Utilizing clinical operational software processes all schedule requests while ensuring maximized productivity of all clinical field staff. Addresses all missed, rescheduled, reassigned, and declined visits.

Maintains communication with patient/caregiver/discharge planner with regard to patient status when hospitalized.

Assist with scheduling on-call and weekend coverage

Receives follow up lab reports and enters lab results in a coordination note in electronic record.

Runs, audits, and follows up on assigned daily reports

Maintains and contributes to the efficiency of operations by consistently complying with all company and regulatory policies, procedures, and guidelines.

Attends meetings and in-service programs as requested.
Have current license as a Licensed Practical Nurse (LPN) in the practicing state.

One (1) year home health care experience or two (2) years experience in a scheduling position in home health care.

Strong communication (written and verbal) and interpersonal skill

Demonstrate the ability to function with supervision and make appropriate judgments.

Strong knowledge of office machines and equipment such as copier, fax, postage meter.

Proficient in use of personal computer and popular software. (i.e. Microsoft Office and above average speed and accuracy)
