Commonwealth Nursing Solutions is a non-medical home care agency providing care to the mentally and intellectually disabled client under Michelle P Waiver and Supports for Community Living.

Job Skills/Requirements
Successful completion of DDID requirements:
Medication Administration / Seizures
Crisis Prevention
Core Training
CPR w/First Aid
Reliable transportation is a must
Must have current driver's license, current auto insurance C.N.A license preferred. All employees required to have clean drug screening and criminal background record.
Home Care Aide client assignments; providing Community Living Supports, personal care, respite, homemaking, transportation, etc.
Must excel at dependability, Successful completion of DDID requirements:

Must excel at dependability, punctuality, documenting and reporting as well as organizational skills.
Must complete all DDID and CNS training requirements
DDID and Home Health experience is preferred.
Job description includes community inclusion/integration,assisting with activities of daily living and personal care including, but not limited to: bathing, mouth care, skin care, shaving, dressing, feeding, positioning, transferring, ambulation, exercise, toileting, medication reminders, vital signs. May include assistance with housekeeping, meal preparation, transportation, escort to medical and other appointments, companionship, and respite.
Additional Information/Benefits
PRN position does not have a regular schedule and is based on associate availability and client needs
A PRN associate must be available to work at least (1) weekend a month, and at least (1) major holiday.
