Under indirect supervision, provides ambulatory behavioral health services including evaluation, diagnosis, treatment planning, case management, and psychotherapeutic intervention provided to individuals, families, groups, and/or Intensive Outpatient Programs. Provides services to adults of all ages, adolescents and children.

Essential Functions:
Gathers information and identifies resource and referral services for patient care
• Conducts preparation and evaluation of psychosocial assessments and development of social work service plans
• Performs case management, coordination, and monitoring of social work service plans in areas of personal, social or economic resources, conditions, or problems
• Assists with administration, development, implementation, and evaluation of social work programs and policies
• Assesses treatment and prevention of psychosocial dysfunction, disability, or impairment, including emotional and mental disorders
• Conducts social work consultation
• Performs clinical diagnosis or psychotherapy
• Applies formal knowledge base, theoretical concepts, specific functional skills, and essential social values used to effect change in human behavior
• Utilizes formal knowledge base, theoretical concepts, and specific functional skills to effect emotional responses, and social conditions; helps individuals, couples, families, groups, and community organizations enhance or restore their capacities for personal and social functioning and preventing and controlling social problems
• Participates in or assists with therapeutic and educational group programs to address needs of patient; evaluates outcomes and effects of treatment for individual patients and programs
• Researches, collects and records data and information relating to identification and treatment of behavioral and interpersonal problems of patient; proposes to psychologist and/or team alternatives for interventions and approaches for resolution of these problems; assists in establishing clinical best practices and guidelines for delivery of service to population of clients served
• Assists and empowers clients to participate in their treatment plan; engages client in therapeutic relationship; educates client regarding mental health services and their benefits; assists members to use services appropriately; makes appropriate recommendations and referrals for patients to obtain information, support, services and assistance they may need from Kaiser Permanente or community resources
• Follows established guidelines and laws for assuring client safety and confidentiality; explains same to clients
• Collaborates and consults with physicians and other interdisciplinary clinical team members to support assessments and decision making; reviews cases or quality assurance concerns with clinical team as appropriate; develops skills with expertise of team members in management of patients
• Collaborates and communicates with primary care staff and others in organization involved with care of patient; acts as liaison between mental health services and primary care, emergency department and other outpatient clinics as requested by psychologist

Basic Qualifications:
Familiarity with conducting group therapy, intensive outpatient treatment, and/or partial hospitalization programs
• Familiarity with writing treatment plans
• Familiarity with conducting assessments and using the DSM IV
• Familiarity with frequently used psychiatric medications
• Graduate degree in social work from an accredited college or university, passing score on the clinical level national board examination, and supervised clinical social work training, as required by Hawaii Revised Statues (HRS) 467E-7
• Valid Hawaii Clinical Social Worker license (LCSW) (must meet education requirement(s) for Hawaii State licensure)
• Current BLS for Healthcare Provider CPR or CPR/AED for the Professional Rescuer Certification
• Demonstrated knowledge of and skill in Adaptability, Change management, Conflict resolution, Decision making, Group process facilitation, Influence, Interpersonal relations, oral communication, problem solving, quality management, teamwork, and written communication
• Talking to co-workers, customers, outside vendors, and on the telephone
• Reading, writing, speaking, and understanding English
• Training/giving and receiving instructions
• Mathematical ability, attention to detail (e.g. organization, prioritization, proofing), concentration, and alertness
• National Provider Identifier (NPI) and Taxonomy code required at time of hire
Preferred Qualifications:
Primary care, chemical dependency, and inpatient work experience
• Consultation/liaison experience
• Demonstrated knowledge and skill in documentation according to national accreditation guidelines
