Responsible for quality case management, supervision and evaluation of an assigned service team. Outreaches and assists in securing linkages for client services in the community. This candidate will supervise and help a team of Senior Service Specialists to set work priorities and maximize agency and community resources in the provision of case management services; Work towards resolution of identified client problems through counseling, case conference, and authorization of DFTA services. Receive documentation and visit reports for completeness and quality of service. Approves initial and ongoing re-assessments, and discharges. Promote utilization of community resources and acts as a liaison with other community agencies providing services.
- Master's degree in Health and Human Services, one year of supervisory experience, and one year of case management experience with geriatric population, and or/persons with a history of mental illness, homelessness, or chemical dependence
- Bachelor’s degree in Health or Human Service, two years of supervisory experience and two years of case management experience with geriatric population, and/or persons with a history of mental illness, homelessness, or chemical dependency
- NYS LMSW, preferred
