Hospice Registered Nurse Case Manager
Homestead Hospice is seeking reliable RN’s to join our growing team in the East Valley.
The primary responsibilities:
Coverage for caseloads, and new admits
Meetings (i.e. IDT, employee & in-services)
Coverage of the East Valley and the Florence/Coolidge area
Collaborate with other departments such as; Nursing, Social Worker and Admin to ensure complete care of patients
Minimum of 3 days of availability per week for PRN
