The Heart Failure(HF) Nurse Practitioner (NP) provides autonomous care to patients with HF in accordance to established advance practice clinical guidelines and within a multidisciplinary team context.A NP is a registered nurse with advanced knowledge, skills, and recognized clinical competence who is appropriately credentialed through a national nursing credentialing organization.A NP is a recognized expert nurse and clinical leader within the specialty and profession who functions at an expert level to ensure the best outcome for the patient. The HF NP is a nurse recognized as expert in cardiac care, having specific knowledge and education within the specialty.Role of the HF NP includes: - Assessment of health status through obtaining a relevant health history and performing a comprehensive physical exam- Identify health needs and referring to appropriate health care providers where required- Recognize early clinical features of worsening HF and implement appropriate interventions to prevent hospital admissions- Prevention and/or management of any complication- Initiate appropriate diagnostic tests- Optimize medical therapy using American College of Cardiology(ACC), American Heart Association(AHA)/ Heart Failure Society of America(HFSA) clinical guidelines for HF as the standard of care- Promote self-management strategies through individualized education of patients (families, caregivers and health care personnel)- Follow patients recently hospitalized with HF( New York Heart Class III-IV), frequent re-hospitalizations and those with multiple, active co-morbidities- Provide ongoing continuing education to health care workers regarding care of HF patients- Lead a developed multidisciplinary Heart Failure Team during weekly patient care rounds- Provide ongoing education for medical and nursing staff- Develop strategies to improve patient education and adherence to medical regimens- Utilize both pulmonary and cardiac rehabilitation programs in order to enhance coordination of patient care- Coordinate care with home care practitioners.- Assist in providing continued palliative and when required hospice care management.Post Graduate Degree (MSN) required.Board Certification required.State of Florida ARNP license required.American Heart Association BLS and ACLS Provider Certifications required.Minimum of 1-3 years clinical experience in cardiology and managing congestive heart failure patients required.Knowledge of state/federal, Joint Commission regulations. Excellent communication skills and collaborative approach for working with all levels within the healthcare team is a MUST!
