UIC, College of Nursing, Biobehavior Health Science Department is seeking Clinical Practice Nursing Educators with emphasis in heart failure management. Open Rank – Non-Tenure Track faculty.
The Clinical Practice Nursing Educator is a master's prepared nurse who demonstrates advanced knowledge and skillls in multiple roles and functions. The Clinical Practice Nursing Educator assumes accountabilty for client care outcomes through the assimilation and application of evidence-based information to design, implemnet, and evaluate patient plans of care. The role integrates teaching, practice and research and includes promotion of wellness, prevention of complications and management of acute and chronic conditions. The Clinical Practice Nursing Educator will practice and provide instruction in various UIC College of Nursing practice sites.
Responsibilities fall into 4 broad categories and may include:
1. Outpatient responsibilities: Frequent contact with patients through phone follow-up during the first 2 weeks following hospital discharge. Evaluate patients in clinic early post-discharge (within 7 days) with HF cardiologist . Tapered contact with patients through phone follow-up and /or coordination of home visits by visiting nurse. Referral to social services, rehabilitation services, home health aide or other behavioral specialists as needed. Coordination and communication with home health nursing to assure an adequate nursing assessment of the home environment, community resources, caregiver support and psychological support. Self-management and coaching support to empower patients to effective manage their HF. Continued patient education in Heart Failure clinic. Provide coverage of heart failure hotline. Monthly reports provided to cardiology and hospital that include information about patient demographics, clinical characteristics, service utilization and patient outcomes. Regular review of the ‘hospital-to-home transition process’ to identify gaps in care process and to ensure continuous improvement across the continuum of care .
2. Establish Heart Failure Hotline: Provide phone number to patients for 24/7 access to heart failure program. During working hours phone calls answered by HF NP and HF PharmD. After hours phone calls answered by cardiology Fellows. HF physicians on call 24/7 to provide support to NP, PharmD and Fellows.
3. Develop Relationship With Home Health Service: Select home health service to provide home visits to HF patients. Maintain regular contact with home health nurse and provide 24/7 access to HF physician.
4. Other duties as assigned : Assist in making follow-up appointments and scheduling procedure. Keep HF database up-to-date. Help maintain contact with referring MD’s.
This position includes a 30% faculty appointment.
