Post by Jobing - Requisition Number 14–0057 Post Date 5/14/2014 Title Behavioral Health Professional–Bilingual City Westminster State CO Clinica Family Health Services (CFHS) is seeking a professional, well–organized, team–oriented individual to fill our open Behavioral Health Professional position at our Federal Heights Clinic. At CFHS, our nationally–recognized healthcare model is designed to care for the whole patient. In doing this, we have integrated dental and behavioral health care into our medical clinics to create small but efficient care teams, or what we call ‘pods'. Each pod focuses on providing quality services to all CFHS patients.

As a member of the integrated care team the Behavioral Health Professional has an opportunity to partner with other providers to encourage and help patients receive the behavioral support necessary to impact their overall health. The Behavior Health Professional is responsible for providing brief intervention and consultation in real time to patients in a primary care setting; managing the assessment, treatment planning and episodes of brief goal focused therapy for established Clinica patients; providing crisis intervention as needed; monitoring mental health referrals to CMHC's for patients with higher behavioral health needs; coordinating care for complex patients with co–morbid mental and physical health conditions; telephone call follow–up for a caseload of high–risk chronic care patients, especially patients with depression; functioning as a group visit team health educator – specifically for patients with chronic pain, depression and anxiety; providing clinical support to the site case managers for prenatal high risk patients and psychosocial issues with chronic care patients; acting as a consultant and resource to members of the care team on skillful and effective interventions with patients with chronic illnesses and mental health diagnoses; assisting with staff training as needed; functioning as chronic care self–management support expert and trainer at clinic sites; meeting minimum documentation requirements for all services rendered in a timely manner; ensuring data collection as appropriate and required; assisting with implementation of an effective Quality Improvement Program; as well as, demonstrating knowledge of the principles of growth and development over the life span Voted by the Denver Post as, ‘One of the Best Work Places in Colorado' our focus is to provide quality healthcare within a fun and progressive work environment. Education / Experience: *Licensed Clinical Social Worker, Licensed Clinical Psychologist related Master's Degree or licensure required within six months of completion of supervision hours as required by the State Licensing Board requirements *Two years experience in community health preferred Knowledge, skills and abilities: *Short term, strategic, goal oriented therapy skills (SFBT) in a CHC or other health care setting *Familiarity with current DSM and ICD Diagnosis Codes *Sensitivity to low income, ethnic minority community *Bilingual in Spanish/English required www.clinica.org EOE
