Hospice of Southern Maine is seeking a full time CNA/HA to provide direct hospice care to our patients in their place of residence. HA certification is preferred but we will provide this training to the right individual!
Essential Duties & Responsibilities:
Works within the scope of practice for HA under the supervision of the registered nurse. Follows the HA Plan of Care to deliver quality patient care, including: bathing; dressing/grooming; s kin care; a ssistance with elimination, nutritional needs, mobility; and homemaking.
Makes and adjusts patient rounds to meet patient and agency needs.
Promotes teamwork by participating actively and appropriately as a member of the interdisciplinary team.
Completes d ocumentation that is legible, and clearly reflects care given and submitted in compliance with timeliness guidelines.
Works with diverse social and economic situations without imposing own values on patient and family. Maintains appropriate boundaries.
Carries out duties and responsibilities of position in a manner that exemplifies excellent customer service.
Demonstrates reliability, dependability and flexibility in providing services.
Participates in weekend/holiday rotation as required.
