POSITION SUMMARY Certified Home Health Aides work as members of the home care team under the direction and guidance of a physician, professional nurse or therapist. The Certified Home Health Aide provides personal care, complex tasks, and light housekeeping in the patient's home. The Certified Home Health Aide provides this care to the following patient population: mainly adults and geriatric patients. The CNA on occasion may need to provide care to other age groups; care will be specifically delegated by a registered nurse.

This care will be delivered in accordance with established standards of performance. All the job standards and skills included within this document will be performed and documented according to established policies, procedures, and guidelines within the Home Health Department and Trinity Hospital. STANDARDS MEASURED BY: Direct or indirect observations, documentation, procedure review, valid complaints, QI reports, and review and demonstration. Any areas found to be below in a standard for more than 1 consecutive year, will result in a below standard rating for the entire standard.

MINIMUM QUALIFICATIONS AND REQUIREMENTS Licenses and Certifications Required Must be certified through the North Dakota State Department of Health. Current drivers license required and car available to travel to patient's home. Educational Requirements High school graduate Experience Requirements Must have previous experience as a nurse's aide. Special Skills or Training Requirements Ability to: Organize time well and meet job expectations and performance standards.

Skill inventory checklist and competency verification completed annually CPR certification every 2 years Demonstrates knowledge for age-specific factors necessary to perform safe and effective performance of duties.
