I. Position Summary: The Medical Social Worker III is responsible and accountable for assessing the psychological, social, emotional, cultural and/or financial needs that impact high-risk patients’ health and recovery. Has the ability to utilize social service, supportive counseling, and clinical knowledge in the assessment of these needs. Collaborates with case management in the development of a discharge plan, and addresses assessed needs in a timely manner. Knowledge of payer source implications on discharge plan and addresses assessed needs in a timely manner. Knowledge of payer source implications on discharge plan, resources and placement options to maximize patients’ successful disposition. The Medical Social Worker performs delegated duties consistent with the scope of practice and functions within the philosophy and overall plan of care at RMC. The Medical Social Worker III displays positive support for the values and mission of RMC.

II. Position Requirements:

A. Licensure/Certification/Registration: Current Colorado LCSW required

B. Education: (MSW) Masters of Social Work required

C. Experience: 3-5 years recent clinical social work experience, experience in acute care setting preferred.

D. Knowledge, Skills and Abilities: Must have the ability to assess information, apply critical thinking processes to determine course of action, and relay this information to the interdisciplinary team. Requires excellent human relations and oral/written communication skills. Must be able to prioritize workload, and develop efficient methods of managing time.

III. Degree of Supervision Required: Minimal; expected to be self-directed.

IV. Orientation Period: Minimum of three weeks training/orientation contingent upon experience. Extensive orientation may be required and will be determined by Department Director / Designee.
