Our progressive community support outreach program is in need of a case manager to provide counseling and case management services to psychiatrically disabled adults and families in order to maintain and improve their level of functioning in the community. Duties include:
Spanish speaking abilities and knowledge of substance abuse a plus. Ability to work across multidisciplinary teams, driver's license, approved driver's record, and reliable transportation required. One year experience in community mental health preferred.
- Provide case management services to clients in their home (residence), at the Center, in the community and other institutions, (such as ACI, hospitals, substance abuse treatment settings.)
- Assess and evaluate all clients on an ongoing basis, providing support services and/or crisis intervention as needed
- Develop, design and implement treatment goals/plan with client
Education Requirements: AA in a human services field required; BA/BSW in a human services related field preferred.
License/Certification Requirements: Valid driver's license. Position requires you to work towards CCSP after 6 months of employment (sponsored by NRICS)
