Mercy Medical Center in Oshkosh has a Cancer Care Navigator (Nurse Practitioner) opening in their Cancer Center. This is a full-time exempt position.
Provides comprehensive assessments, care coordination, and care planning to patients as necessary in collaboration with Primary Care Provider - improving coordination, continuity, assessibility and appropriate utilizaton of health care services.
In absence of collaborative practice agreement, but still under supervision of a physician, performs exams and consults with Primary Care Physician regarding treatment orders and diagnostic testing.
Both in collaborative and non-collaborative settings, NP will maintain ongoing communication with Primary Care Physician as indicated.
Through a variety of media responds appropriately to patients' questions and provides consultation to them as necessary; assesses patient' needs, coordinates resources.
Performs all documents all activities in accordance with established policies and protocols.
Masters prepared in Nursing. Licensed as a registered nurse in State of Wisconsin. Certification as Nurse Practitioner by American Nursing Association.
Previous work experience preferred as a Nurse Practitioner 2-4 years, adult or geriatric experience preferred.
Valid Wisconsin drivers license
Computer skills, excellent verbal and written communication skills, Excellent problem solving and decision making skills. Ability to work independently and proritize work.
Must possess a strong understanding of case management and evidenced based clinical intervention.
