Position Details:
This position is responsible for case follow-up of all analytic patients for the Division hospitals.

DUTIES INCLUDE BUT ARE NOT LIMITED TO:
Routine mailings of follow-up letters to physicians, patients and health care facilities.

Phone calls are required when a timely response has not been received. Time parameters and phone script to be provided by Cancer Data Team Lead.

Ensure case follow-up of 90% of analytic patients at each facility is completed within six months of first contact to remain compliant with ACOS standards.

Ensure an 80% follow-up rate is maintained by facility for all eligible analytic patients from the reference date in accordance with ACOS standards.

Ensure a 90% follow-up rate is maintained by facility for all analytic patients diagnosed within the last 5 years, or from the registry reference date, whichever is shorter in accordance with ACOS standards.

Attend state and national educational activities as approved by Director.

Participates in required continuing education programs and annual reorientation.

Other duties as assigned

Qualifications

KNOWLEDGE, SKILLS & ABILITIES

Knowledge of disease index and pathology reports.

Computer skills, including word processing, data input, Meditech and other required software.

Communication skills, including phone etiquette and written correspondence

EDUCATION

High school degree or GED

PHYSICAL DEMANDS/WORKING CONDITIONS

Requires prolonged sitting, some bending, stooping and stretching. Requires eye-hand coordination and manual dexterity sufficient to operate a keyboard, photocopier, telephone, calculator and other office equipment. Requires normal range of hearing and eyesight to record, prepare and communicate appropriate reports. Requires lifting papers or boxes up to 50 pounds occasionally. Work is performed in an office environment. Work may be stressful at times. Contact may involve dealing with angry or upset people. Staff must remain flexible and available to provide staffing assistance for any/all disaster or emergency situations.
