Graduate from a Registered Professional Nursing Program
Responsible for integrating, facilitating and coordinating patient care for high-risk patients across the healthcare continuum. Ensures the delivery of cost effective, outcome oriented and quality care within an appropriate LOS. Manages patient care to prevent fragmentation and duplication of services. Works in collaborative practice with the physician and other members of the health care team to meet patient specific and age related needs linking cost and quality to patient care.
Baccalaureate prepared RN or Social Worker or CCM or eligible to sit for CCM or two years acute care/home care and/or case management experience preferred. Masters Degree preferred.
Ability to function autonomously maintaining a high level of clinical and professional accountability. Demonstrates skill in creative problem solving, facilitation, collaboration, coordination and critical thinking. Embraces change and continuously identifies opportunities for improvement by demonstrating a commitment to creativity and innovation.

Computer literacy and data analysis skills are required.
