Care Manager/Substance Abuse Counselor, HIV/AIDS Scatter Site, Bronx

We are looking for a Case Manager/Substance Abuse Counselor for our Scatter Site Program in the Bronx for persons living with HIV/AIDS.
The Care Manager/Substance Abuse Counselor is responsible for:
  • Ensuring that each client is receiving appropriate and effective case management services.
  • Conducting comprehensive assessments and developing comprehensive service plans that identify mutually agreed upon long and short-term goals.
  • Conducting face to face contacts (home visits/office visits) as specified in the service plan(s); with a minimum of one home visit per month and one contact per week.
  • Providing alcohol and other substance abuse services and referrals; assisting in accessing primary medical care; assisting with adherence to medication regiments; teaching activities for daily living; nutritional services; recreation programs; assistance in employment assessments, training, and private sector placement services; prevention with positives; and other necessary services to clients to increase their capacity for independent living. Also monitoring and ensuring clients are linked to needed services.
  • Reviewing the case management service plans and conducting reassessments on a quarterly basis to ensure that services are appropriate for the clients’ current needs.
  • Providing crisis intervention to clients and their families as needed.
  • Maintaining ongoing awareness of clients’ medical conditions so that services are appropriate to changing needs.
  • Arranging support and home health care services as necessary, making referrals to medical, home care, mental health and/or substance abuse services
  • Ensuring that clients pay the balance of their rent, telephone and utility bills.
  • Providing counseling and orientation regarding tenant’s rights and responsibilities to maintain and retain housing.
  • Assisting clients in moving from hospitals, nursing homes or other locations into their apartments, and, providing transportation when necessary.
  • In collaboration with vocational counselor and entitlement specialist providing advocacy & referrals to employment programs for training and job placement in the private sector.
  • Assisting clients in locating alternative housing options, when a client’s health so requires.
  • Providing and coordinating all supportive services as needed.
  • Collaborating with all providers within and outside of Comunilife involved with the client/family. Coordinating and collaborating with HASA personnel as required.
  • Maintaining regular personal contact, developing a trusting working relationship with clients in order to monitor physical and emotional capacity for independent living
  • Maintaining required documentation including all contacts made with the client and any contact on their behalf
  • Submitting a weekly schedule of field visits
  • Participating in mandated training workshops, seminars, and case presentations as well as meetings with peers and supervisors
  • Conducting initial assessments including substance abuse assessments and in collaboration with case manager assisting in the development of comprehensive service plans to identify mutually agreed upon long and short-term substance abuse related goals.
  • Providing substance abuse counseling and support services to clients, either in the client’s home or office; as specified in the service plan(s); to monitor the physical and emotional condition of the client.
  • Conducting weekly substance abuse groups.
Knowledge and Skills:
  • Extensive knowledge of case management and alcohol and substance abuse principals and practices.
  • Initiative and ability to learn HRA-HASA Scattered Site Program Desk Guide and HIV/AIDS Program, Office of Mental Health regulations.
  • Responsible, organized, effective, and efficient. Ability to perform multiple assignments simultaneously. Communication skills (effective & Actively listening, Verbal and written) Community minded, patient, creative, flexible, compassionate, and culturally sensitive to persons who are diagnosed with HIV/AIDS, severe and persistent mental illness or substance abuse.
  • Ability to deal tactfully and diplomatically with other Comunilife employees, Federal, State, and City officials, professional and technical groups, clients, and the general public.
  • Extensive knowledge of community resources
  • Ability to perform within a team and accomplish multiple assignments simultaneously.
  • Communication skills (effective /active listening, verbal and written)
  • Ability to deal tactfully and diplomatically with other Comunilife employees, Federal, State, and City officials, professional and technical groups, clients, and the general public.
Education and Experience Requirement(s)
Bachelors Degree required; with minimum one to two years field work experience HRA/HASA and substance abuse direct work experience. Individual, family and group counseling experience required, particularly with HIV+/AIDS affected populations and/or substance abuse individuals. Preference given to persons with experience in scattered sites housing programs, bi-lingual, bi-cultural in Spanish, a plus.
