Care Manager/Behavioral Health Provider

General Description of Duties:

The Care Manager/Behavioral Health Provider is responsible to provide behavioral health and care management services to children, adolescents, adults, couples and families. The Care Manager/Behavioral Health Provider will coordinate the care of SALUD patients that are stratified as high risk, with emphasis on those enrolled in the Accountable Care Collaborative (ACC). The aim is to improve patient health and reduce health care costs through the development of comprehensive care plans that address the medical, social and behavioral needs of each patient. Additionally, the Care Manager/Behavioral Health Provider will provide integrated behavioral health (mental health and behavioral medicine) services to primary care patients in a primary care setting.

The Care Manager/Behavioral Health Provider will work closely with SALUD patients, SALUD clinics, local community hospitals, community members and partnering agencies. The Care Manager/Behavioral Health Provider will work in a team-based approach, interacting with various staff members across departments, as well as supporting patients and their families to facilitate self-management, self-efficacy, and behavior change. The Care Manager/Behavioral Health Provider will adhere to the spirit of the SALUD mission statement while performing assigned duties.

Supervision Received:

Works under the direct supervision of the Director of Integrated Services and Psychology Training and the Accountable Care Manager.

Specific Duties:

Assist in risk stratification of high risk Salud patients, with an emphasis on patients enrolled in the ACC.

Assess patients, medical, dental, behavioral health and social needs.

Conducts mental health screenings utilizing standardized instruments.

Determine appropriate level of behavioral health treatment in accordance with relevant ethical and legal standards.

Provide behavioral health and care management services appropriate to the age-specific needs of all patients in accordance with the principles of growth and development for infant, child, adolescent, adult and geriatric life cycles.

Provide PCP initiated consultation services that may include differential diagnosis, psychoeducation, brief intervention, referral for further treatment etc.

Develop comprehensive care plans for patients with referrals to internal and external resources.

Manage psychosocial aspects of chronic and acute diseases.

Assist patients with applying and accessing assistance programs.

Other duties as assigned.

Qualifications for Appointment

Knowledge, Skills, and Abilities:

Self-starter, able to work independently and flourish without extensive supervision.

Excellent working knowledge of behavioral medicine and evidence-based treatments for medical and mental health conditions preferred.

Ability to work through brief patient contacts as well as to make quick and accurate assessments of mental and behavioral health conditions.

Detail oriented, able to coordinate and prioritize multiple responsibilities.

Excellent organizational, data management and documentation skills.

Ability to collaborate with an interdisciplinary healthcare team including primary care providers, medical support staff, behavioral health workers and other healthcare professionals.

Knowledge of community resources and application process.

Good verbal and written communication skills.

Computer knowledge and skills including proficiency in Microsoft office programs (Access, Excel, PowerPoint, Word).

Willingness to learn new software programs and work processes.

Working knowledge of office equipment such as copiers, scanners, digital cameras, fax machine, etc.

Commitment to working with the underserved.

Sensitivity to low income and ethnic minority community a must.

Bilingual English/Spanish required.

Education:

Masters degree in psychology, counseling, clinical social work, psychiatric nursing or equivalent.

Experience:

Experience in care management, patient navigation or social work experience preferred.

Experience in a community health setting or with underserved populations preferred.

Licensure/Certification:

Licensed as a mental health provider by the State of Colorado.

Compensation:
