The Case Manager Nurse is responsible for care coordination, discharge planning, and utilization review. The RN Case Manager will facilitate the interdisciplinary plan of care with a focus on evaluating the appropriateness of clinical care, medical necessity, admission status, level of care and resource management. The RN Case Manager is responsible for maintaining interdisciplinary communication through daily formal and informal consultative activities with hospital staff, patient/patient representative, patient family as authorized by patient, physician and community resources as needed to facilitate safe discharge and appropriate resource management. The RN Case Manager will coordinate communication of clinical information through facility UR system to insurance companies or reviewers as required acting as patient advocate while understanding federal and state regulations as medically indicated per patient and maintain patient confidentiality. RN Case Management is a physician based model and includes patients of all ages.
Required Job Qualifications include:
- Current State of Alaska licensure as an RN required
- Minimum of 3 years clinical experience in nursing required
- American Heart Association BLS healthcare provider certification required
- Bachelor's of Science in Nursing preferred
- CCM preferred
- Experience in Utilization Review, Discharge Planning or Case Management preferred
