We are currently seeking a Resolve Application Specialist/Case Manager to work in our Salt Lake City hospital location. This exciting position is responsible for providing high quality client focused service by using knowledge of state and federal government program requirements to efficiently maintain and update general information for our patients. The position is fulltime Monday through Friday and the hours are 8:00 AM to 5:00 PM. Some of the duties include:

  • Documenting and updating the computer system based on daily contact with an assigned applicant group
and application evidence requirements (work activity, medical condition, third party

contacts, demographics, employment or salary verification, etc)

  • Reviewing inbox for daily work queues and following them to completion
  • Answering incoming calls from hospital patients and our hospital staff
  • Completing required forms to support the application process
  • Answering incoming calls from agency caseworkers and providing updates regarding application or evidence
status

  • Assisting inquiries from patients and family representatives regarding the application process
  • Assisting patients with scheduling appointments with caseworkers and/or attending appointments
REQUIREMENTS:
  • Associates Degree
  • Knowledge of State Medicaid programs (CHIP, MN, ABD, Newborn, etc) and/or federal SSI/SSDI
disability application processes and programs

  • Able to read and speak both English and Spanish fluently
  • Previous case management experience
  • Previous customer service experience, preferably in a call center or office environment
  • Strong and demonstrated verbal and written communication skills
  • Demonstrated experience working with people of various educational and socioeconomic backgrounds
  • Computer skills including data entry and word processing
  • Knowledge of medical terminology
  • Proficient computer skills in Microsoft products (Outlook, Word, Excel)
  • Strong organizational skills including the ability to prioritize plan and follow through
  • Professional and courteous phone demeanor
  • Skill in identifying problems as well as researching and recommending solutions
  • Reliable transportation; some travel to local agencies may be needed
PREFERRED:
  • Bachelor's Degree, preferably in Social Work, Psychology or Business
  • Experience communicating with government case workers and/or hospital personnel
  • Previous experience in a health care environment
  • Previous experience working for Social Security Administration (SSA), Department of Public Health and/or
Office of Public Assistance

  • ACA Certified Application Counselor, Navigator or Assister
