- Position Description:
Professional nurse who assumes accountability and responsibility for managing the health needs for children with special health care needs covered by the CMMI collaborative agreement. Facilitates coordination of care by all members of the health care team to facilitate achievement of appropriate outcomes and CHMCA initiatives. Tracks all financial expenses incurred managing the health needs for children with special health care needs in collaboration with the grants administration. Ensures accurate data entry for all grant activities and research initiatives. This nurse will be completely dedicated to the CMMI/CHMCA collaborative agreement.
Graduate from an accredited school of nursing, licensed and currently registered as a professional nurse in the State of Ohio. Bachelor of Science in Nursing required; Masters Degree in Nursing preferred. Previous case management experience preferred. Must possess excellent oral and written skills. Demonstrate leadership and interpersonal relationship skills which enhance communication and promote conflict resolution. Demonstrates ability to communicate effectively with parents and the interdisciplinary team. Current Health Care Provider BLS training is required. Experience in managing health needs for children with special/complex health care needs.
