I . Education:RN required; BSN strongly preferred.MSN highly desirable.MSW with extensive prior case management experience considered.
II. Licenses/Certifications:Must have an active Virginia license.BLS Certification required.Certification in CM or Quality is desired.
III. Experience:Must be an assertive, self-starter capable of functioning autonomously. 2 years of prior case/quality/utilization management or care coordination experience required. 3+ years of prior clinical experience required, preferably in ED, PACU, or critical care.Prior MS Office experience required; previous MIDAS experience desirable.
Assess the physical, functional, social, psychological, environmental, and financial needs of patients within an assigned area. Identify a comprehensive plan to meet the patients' service needs, and coordinate the delivery of high-quality, cost-effective care across the continuum. Facilitate clinical decision-making that not only takes into account the patient's clinical condition, but also the medical necessity of the proposed treatment, conformance to state and federal regulatory standards, and third-party payor requirements. Depending on the assigned unit, the Case Management core functions of Utilization Management, Care Coordination and Discharge Planning may be the responsibility of a single Case Manager, or the responsibilities may be shared by a team of Case Managers.
