Full-time position in our Community Support Program.

Requirements/Qualifications include:
A minimum of a Bachelor's degree in counseling and guidance, rehabilitation counseling, social work, education, vocational counseling, psychology, pastoral counseling, family therapy or related human service field from an accredited institution is required.
Must be 21 years of age to meet Medicaid billing requirements.
Valid driver's license, current automobile insurance, and reliable transportation to be used on the job is required.
Previous experience working with individuals with severe and persistent mental illness is preferred.

Job Responsibilities include:
Provides case management, community support, assessment, treatment planning, and crisis services as appropriate.
Maintains on-going relationships with law enforcement, government, housing providers, referral sources and other community.
Assists clients in accessing specialized services by identifying appropriate referrals, providing information to referral programs within the Community Elements continuum or from other organizations
Collects and documents financial information necessary to verify client eligibility to receive grant funded services.
Provides appropriate referral and linkage to internal and community resources based on client needs.
Documents all services provided in written and/or electronic format.
