3 to 5 Years of Experience Required
The Family Support Services case manager will work between two hospital departments, the Primary Care Clinic and Early Intervention Services. This position will be responsible for providing training and consultation to Primary Care clinic staff and family navigators regarding resources for young children with developmental/behavioral/family support needs. In addition the positon will provide family navigation, referral and community liaison support to children and families that are part of Early Intervention Services (EIS) patient population. EIS duties will include intake and phone contact with resource and referral sources in the community, direct phone or clinic contact with families, and support/consultation with direct service providers.
Bachelors Degree with at least 4 years of related experience. Masters degree in Social Work or Child Development preferred.

Knowledge, Skills & Abilities:
Normal and atypical child development. Case Management skills, including knowledge of community resources. Ability to work effectively in a multidisciplinary setting with a diverse group of professional and support staff. Ability to effectively and sensitively engage families. Capacity to gather information regarding family history, child behavior problems and development, and parental concerns. Comfort in discussing potentially sensitive areas of need, including food, housing, medical concerns, and behavioral issues associated with violence, trauma, and substance use. Experience with EMR or other medical record systems. Computer skills related to database input and management. Excellent verbal and written communication skills and problem solving skills. Ability to work under pressure and respond effectively to crises. Spanish Speaking capacity highly advantageous based on patient population needs.
