The goals of the Functional Family Therapy Case Worker is to prevent child abuse and neglect; keep families together; stabilize and maintain family functioning; strengthen families and reunite children with their families.
Under the direction of the Social Work Supervisor, the Case Worker shall serve as primary service provider. Due service mandates, the Case Worker shall be expected from time to time to work evenings. The principal tasks of the position include:
1. To provide direct service utilizing the Caseworker, Framework Principals (Engagement, Assessment, Change, & Safety) to individuals, families and groups with mental health and substance abuse issues.
2. To carry out ongoing risk and safety assessments of the family and clearly document these assessments in their progress notes. Bring al the risk/safety concerns to the immediate attention of their supervisor.
3. To provide services utilizing the full range of in-house and community-based services to address issues that place a child at risk of foster care placement and promote maximum safety, supervision and support for children and their families.
4. To complete diagnostic intake assessments and complete all forms relating to cases which are accepted for services and referrals for families. Family outreach occurs within 48hrs of receiving the case and is signed within 30 days.
5. To enter their progress notes in CONNX in a timely manner (as defines by the department) of the occurrence of the event. Submit hard copies of their progress notes in raft form on a weekly basis to their supervisors. Quality of progress notes must be informative, descriptive and purpose driven.
6. To enter all case work contacts in PROMIS in a timely manner (as defines by the Department).
7. To complete all sections of the FASP illustrating efforts and activities on behalf of the family, family’s behavioral changes, resistance, observation of care take and children, developed in collaboration with the family. All FASP will be submitted to the supervisor according to Department Timelines.
8. To make required home visits per month and collateral contacts, as set by the Department in order to meet ACS mandates and ensure the safety of the children and family. Submit weekly/monthly calendar of scheduled visits to the supervisor. Submit casework contact sheets weekly to supervisor.
9. To facilitate family team conferences for your caseload.
10. To prepare timely, complete and accurate reports, this includes ACS reporting, intra-agency reporting, data collection regarding services rendered and service outcomes.
11. To participate in mandatory once a week supervision and attend all mandatory trainings and general staff meetings.
12. To serve on committees as assigned and perform other job related tasks and duties as assigned.
13. Family outreach occurs within 48 hrs of receiving the case and is signed within 30days. Contact ACS referral to complete JHV in 5 days to complete JHV/ transition meeting and schedule intake within 10 days.
Mas ter of Social Work degree plus experience with troubles families/children in a human service agency setting.
