BASIC JOB FUNCTION:|
To Find and engage families as well as work with youth and their caregiver/ family members in the home to build lifetime connections and strengthen family relationships.
The case manager will assist in family search and engage in order to connect youth with family members and significant others who may be able to provide support and a network of lifetime connections.
The case manager will work with youth and families in the home developing strategies and contracts to enhance communication among family members and clarify expectations about roles, rules, and responsibilities.
The case manager will collaborate with the family advocate in providing linkages to community resources for the youth and the family/ caregiver.
Education/Degree: Minimum BA/BS in Human Services
Years of Experience : Two years
Other skills and requirements: CPR, Mandated Reporter training
