This is a Parttime position.
Bachelor’s degree required. Two (2) years post graduate experience working directly with population. QP status. Flexibility to work irregular and on-call hours. Valid driver’s license and proof of insurance. Clear background check. Organized, detailed-oriented, proficient and timely in all documentations. Ability to multi-task.
Coordinate, monitor and supervise therapeutic foster care placement services for children. Coordinate placements and maintain files. Coordinate admission and discharge planning. Provide weekly home visits. Supervise and monitor service delivery. Assure foster homes are in compliance with State, Federal and agency policies, procedures and guidelines. Support children, families, foster parents and contract agencies. Provide timely and accurate documentation and billing. Ensure confidentiality. Respond to family crisis. Share in after hour and weekend on call rotation. Provide Case Management services for children in foster care, including monitoring, linking and coordinating services and development and maintenance of Person Centered Plans. Other duties as assigned.
