Education and Training : High school diploma or equivalent.
Experience : Basic health care or related record keeping experience preferred but not essential. Retrieves discharged patient records from nursing floors and ancillary departments. Assembles inpatient, emergency room, outpatient surgery, observation, and urgent care patient records in established permanent file order. Analyzes records for completion based on standards set by the Medical Staff and the JCAHO. Updates and maintains discharge lists and quality assurance logs. Organizes and files fetal monitor strips. Attaches loose/late lab reports or other medical reports. Files incomplete records in physicians' incomplete record boxes. Reanalyzes records following transcription and completion by the physician. Completes the weekly delinquent record function as detailed in the department procedure. Interacts on a daily basis with physicians concerning their incomplete records. Prepares the weekly off-staff list. Responsible for the maintenance of all patient files, creating folders, pulling records and microfilm for readmissions, completing quality assurance review on files, retrieval of records for study and committee purposes, and release of information. Reviews all outside requests for medical record information for adequacy of the patient authorization as outlined in the Release of Information policy. Retrieves records for copying. Determines charges and compiles bill for each record released. Maintains a charge/payment log, writes receipts of payment for records requested, and itemizes checks on bank deposit slip for Accounting Department. During periods of vacation or illness in the department will cooperate with other clerical staff members for coverage of all clerical duties. Will be expected to answer phones, aide in the retrieval of readmission records, and handle walk-in and telephone requests for medical information. Responsible for the organization and preparation of medical records for microfilming. Maintains an updated log of all records that are being or have been microfilmed. Verifies quality, accuracy and completeness of all film after processing. Is responsible for shredding of completed microfilmed records. Other duties as assigned by the Director.
