All Valley Home Health Care is seeking compassionate and driven individuals that want to grow with the company. We are currently hiring for Client Care Coordinators for our Phoenix (between 43rd ave and Scottsdale Rd) and East Valley (all clients east of Scottsdale Rd) Territories.
As a Client Care Coordinator, you will be working with another team member. You both will be responsible for managing the care of approximatly 200 terminally ill, disabled, and elderly clients.
Job duties consist of meeting with new clients, developing a care plan, and coming back to the office to find the right caregiver for the client. Your team is also responsible for conducting in- home supervisory visits, personally introducing caregivers to clients, being "On-Call" every other week (taking all calls before 830a and after 5pm and alternating the weekends), contributing to the growth of the company by networking and marketing, and generally being "hands on" for all aspects of all of our client's care. We do have Spanish speaking clients and caregiver's, so being fluent in Spanish is a plus.
This is a FAST PACED and DEMANDING work environment that requires critical thinking and great customer service. You will be required to do both office work and field work.

- BA Degree, 3 years of college courses, or at least 2 years of Case Management/Management Experience
- Expierence in Health Care is a Plus
*Ability to multi-task
*Strong computer software skills
*Must be able and willing to be "On Call" 24-hours a day every other week and make intelligent, caring decisions with a sense of urgency. Bilingual a plus!
