Description The intent of patient navigation activities is to ensure that WHN/MHP members are able to access culturally and linguistically appropriate services in a timely and cost effective manner. The patient navigator will work to overcome obstacles and guide WHN/MHP members through the healthcare system. The Patient Navigator, at the direction of the clinical treatment team and in direct collaboration with the patient, identifies patients in need of navigation and then designs and implements a care management plan that addresses individual patient barriers to care. Under the supervision of the treatment team, the navigator coordinates all aspects of patient care. These activities require the patient navigator to provide documentation in the electronic medical record, and to be knowledgeable about community resources, including the financial, educational, social, and emotional support services available to cancer patients. The Patient Navigator shall demonstrate cultural competency with respect to the patient populations served and will track patients through completion of all diagnostic and treatment plans to ensure timely receipt of care.
High school diploma or equivalent. College degree preferred.
6 months – 1 year experience working closely with patients as an advocate, educator or related experience.
Experienced in serving the population(s) that will be referred for patient navigation services.
Experienced in developing strategies to overcome barriers that prevent clients from accessing services and completing services.
F amiliar with the funded agency and its resources.
Knowledge of both community and healthcare system resources.
Proficient in English (oral and written communication skills).
Bilingual or multi-lingual skills (beyond that of English) appropriate to the patient population served (Haitian Creole), is highly desirable.
Cultural sensitivity and comfort with a wide range of social, racial and ethnic populations
Excellent interpersonal skills to instill confidence and to advocate for patients and their family.
Exceptional organizational skills and ability to keep well written documentation
Knowledge of computers/PC's and a variety of software applications including word processing, spreadsheets, e-mail and other compatible applications; ability to learn additional software packages.
Completed MDPH required Patient Navigation Course within the first 12 months of hire
